A 59-year-old patient underwent an ultrasound scan because of back pain on the lower right side. A hypoechoic homogeneous lesion was incidentally found at the splenic inferior pole. The lesion appeared stable (5.7 cm) when compared with previous ultrasound examinations. Even though it was hypothesized a splenic hemangioma, which has no malignant potential, patient underwent splenectomy 2 years later. The histological report confirmed the hypothesis.
