Very elderly patient (94 years old, female) admitted for abdominal pain and fever. Transabdominal ultrasound revealed acute cholecystitis with direct communication with a liver abscess. Real-time examination shows bidirectional flow of mucopus through the fistula (see video https://youtu.be/WnJC9ELmblA ). She was treated with transgastric cholecystostomy and percutaneous drainage of the liver abscess. She then survived the acute episode and after six months the drain was removed.
