86-year-old man, affected by myelofibrosis with splenomegaly on oncocarbide therapy (poor compliance with hematological controls), presented with hematemesis and melena. CT showed splenomegaly, thrombosis of the splenic vein and perigastric collateral circulations.
He underwent endoscopic evaluation and sclerosis (GLUBRAN) of the gastric fundus varices, complicated by temporary intraprocedural bleeding. After orotracheal intubation we continued the treatment and found good control of haemostasis, stabilized with a fourth injection of GLUBRAN.
The 48-hour check-up confirmed the stability of the condition and the patient survived the acute episode.
